Important Disclosures

Last updated August 12, 2026

Please read these disclosures before you submit your application. They summarize the terms, your rights, and the authorizations that apply when you apply for a Horicon Bank credit card. The rate and fee table shown on the application page is part of these disclosures and varies by the card you selected.

Important information about opening a new account

To help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account (USA PATRIOT Act).

What this means for you: when you apply, we ask for your name, address, date of birth, and other information that allows us to identify you. We may also ask to see your driver's license or other identifying documents.

Authorization to obtain a credit report

By submitting your application, you authorize Horicon Bank to obtain a consumer credit report and other information about you from consumer reporting agencies to evaluate your request, and to obtain updated reports for account review, renewal, and collection purposes if your account is opened.

If your application is denied or the terms are less favorable than requested based in whole or in part on information in a consumer report, you have the right to a free copy of your report from the reporting agency identified in the notice we send you, and the right to dispute the accuracy or completeness of any information in that report.

Consent to electronic records and signatures (ESIGN)

You consent to receive this application, the account agreement, Truth in Lending disclosures, privacy notices, and other communications electronically, and to sign electronically under the federal ESIGN Act. Your electronic signature has the same legal effect as a handwritten signature.

To access and retain these records you need a device with a current web browser, internet access, and the ability to view and save PDF documents. You may withdraw your consent or request a paper copy at no charge by calling us at 888-343-3040 or 920-485-3040 or writing to us at 326 E Lake St, Horicon, WI 53032.

Summary of account terms

Interest rates and interest charges, and the fee schedule, are shown in the Truth in Lending summary on the application page for the card you selected. Rates that vary do so with the Prime Rate published in The Wall Street Journal. Your actual rate, credit line, and any required security deposit are determined by our review of your application and credit report and will be disclosed to you before your first use of the account.

How we calculate interest: we use the average daily balance method (including new purchases). You can avoid interest on new purchases by paying your statement balance in full by the payment due date each billing period (the grace period). There is no grace period on cash advances or balance transfers; interest accrues from the transaction date.

Minimum payment: your minimum payment each period is the greater of a fixed dollar amount or a percentage of your balance plus billed interest and fees, as set out in your account agreement. Making only the minimum payment increases the interest you pay and the time to repay.

Military Lending Act

Federal law provides important protections to members of the Armed Forces and their dependents. If you are a covered borrower, the cost of consumer credit to you and your dependents may not exceed an annual percentage rate of 36 percent, calculated to include costs associated with the credit as required by the Military Lending Act. To hear this disclosure and a description of your payment obligation, call us toll-free at the number on your application.

Your billing rights

The federal Fair Credit Billing Act protects you if there is an error on your statement. If you think there is an error, write to us at 326 E Lake St, Horicon, WI 53032 within 60 days after the error appeared on your statement. We must acknowledge your letter and investigate. While we investigate, you do not have to pay the amount in question, and we cannot report you as delinquent on that amount. A full statement of your billing rights is included in your account agreement.

Equal Credit Opportunity Act notice

The federal Equal Credit Opportunity Act prohibits creditors from discriminating against credit applicants on the basis of race, color, religion, national origin, sex, marital status, age (provided the applicant has the capacity to enter into a binding contract); because all or part of the applicant's income derives from any public assistance program; or because the applicant has in good faith exercised any right under the Consumer Credit Protection Act. The federal agency that administers compliance with this law concerning this creditor is the appropriate federal banking regulator.
